Academic Concession Request Form

This form is for disability-related concessions.  If your request is for a non-disability-related reason, please contact your faculty.

Types of Concession:

  1. Late Withdrawal (W):

    Likely missed the majority of classes and/or coursework. W notation on transcript with date withdrawn noted. Neutral toward GPA.

  2. Standing Deferred (SD):

    Must be in satisfactory standing in the course (attendance and performance) to be eligible. Typically, all coursework has been completed except for the last item (e.g., final exam or paper). Please consult with your instructor to confirm you are academically eligible for an SD.

  3. In-Term Concession (ITC):

    Unable to write a midterm or meet an assignment deadline and require instructor consideration mid-way through the term. To avoid delay, we recommend you connect with your instructor directly to request consideration and email copy your primary Accessibility Advisor.
